What This Code Covers
D7283 covers the surgical exposure of an impacted tooth and the placement of an orthodontic device (typically a bracket and chain) to support its eruption into the dental arch. This is an "expose and bond" procedure where the surgeon uncovers the tooth, bonds a bracket to it, and attaches a chain or elastic that the orthodontist will later use to guide the tooth into position. This is most commonly performed on impacted maxillary canines.
Billing Guide
Bill this code when:
- An impacted tooth is surgically exposed AND a bracket/chain is bonded in the same procedure
- The bracket and chain are placed to support orthodontic traction
- The tooth is being preserved and guided into position (not extracted)
- The procedure combines surgical exposure with attachment placement
Do not bill this code when:
- Only the surgical exposure is performed without bracket placement. Use D7280
- The tooth is being extracted. Use D7220-D7241
- The bracket is placed at a later visit (not during the surgical exposure)
- The orthodontic treatment itself is being billed (separate D8000 series codes)
Insurance and Denial Prevention
Key Payer Rules:
- D7283 is a combined procedure (exposure plus device placement)
- Some payers do not have D7283 in their fee schedules and may require separate billing of D7280 plus the device
- The orthodontic bracket and chain cost is included in D7283
- Do not bill D7280 and D7283 together on the same tooth

- Not in fee schedule - If the payer does not recognize D7283, consider submitting D7280 with a narrative describing the bracket placement. Check with the payer on preferred billing.

- Orthodontic treatment not covered - D7283 is an oral surgery procedure, not an orthodontic benefit. It should be processed under surgical benefits even if orthodontic treatment itself is not covered.
