Bill this code when:
- Comprehensive (full) orthodontic treatment is provided during the mixed dentition stage

- The patient has both primary and permanent teeth
- Full-arch braces or aligners are used
Do not bill this code when:
- Only a limited/specific problem is being treated. Use D8020
- The patient has full permanent dentition (adolescent). Use D8080
- The patient is an adult. Use D8090
- Only primary teeth are present. Use D8040 for comprehensive primary (rare)
