D1551Re-cement or Re-bond Bilateral Space Maintainer - Maxillary

2026 Billing Guide

Re-attaching a fixed bilateral upper space maintainer that has come loose from one or both anchor teeth.

What This Code Covers

D1551 covers the re-cementation or re-bonding of a fixed bilateral space maintainer on the maxillary (upper) arch. When bands or bonded attachments loosen on one or both sides, the appliance needs to be cleaned, checked for fit, and re-cemented to restore its function. This code does not include fabrication of a new appliance or repair of a broken wire.

Billing Guide

Bill this code when:

  • A previously placed fixed bilateral maxillary space maintainer requires re-cementation
  • One or both cemented bands have loosened from the anchor teeth
  • The existing appliance is intact and only needs to be re-attached

Do not bill this code when:

  • The appliance is on the mandibular arch. Use D1552
  • A unilateral space maintainer is being re-cemented. Use D1553
  • The appliance is broken and needs repair or replacement. Use D1516 for a new appliance
  • A new space maintainer is being fabricated and cemented for the first time. Use D1516

Insurance and Denial Prevention

Key Payer Rules:

  • Most plans cover re-cementation as part of preventive benefits
  • Frequency limits may apply (e.g., once per 6-month period)
  • Some payers bundle re-cementation into the original space maintainer fee for a limited period after placement
  • Re-cementation should not be billed at the same visit as initial placement

Common Denials and How to Respond:

  • Bundled with original placement - If the re-cementation occurs within the payer's bundled period (often 6-12 months), the denial may stand. Document the clinical need and separate date of service.
  • Frequency exceeded - If re-cementation is needed frequently, document the clinical reason (orthodontic forces, patient habits) and appeal.
  • Should be D1553 - Verify that the appliance is bilateral on the maxillary arch. D1553 is for unilateral space maintainer re-cementation.
