What This Code Covers
D9219 covers the pre-anesthesia evaluation performed to assess a patient's medical status and suitability for moderate sedation, deep sedation, or general anesthesia. This includes reviewing medical history, current medications, airway assessment, and determining the appropriate anesthesia plan. This evaluation is separate from and prior to the actual administration of sedation or anesthesia.
Billing Guide
Bill this code when:
- A comprehensive pre-anesthesia evaluation is performed before sedation or general anesthesia
- The evaluation includes medical history review, airway assessment, and anesthesia risk classification (ASA status)
- The evaluation is performed on a date separate from the anesthesia administration
- The provider documents a formal anesthesia plan based on the evaluation
Do not bill this code when:
- The evaluation is performed on the same date as the sedation/anesthesia and is considered part of the anesthesia service
- A routine oral evaluation is performed. Use D0120 or D0150
- The patient is only receiving local anesthesia (D9210-D9212)
- The evaluation does not result in a formal anesthesia plan
