What This Code Covers
D9243 covers each additional 15-minute increment of intravenous moderate (conscious) sedation/analgesia beyond the first 15 minutes (which is covered by D9239). Billing Guide
Bill this code when:
- IV moderate sedation extends beyond the first 15 minutes (D9239)
- One unit is billed for each additional 15-minute increment
- The same level of moderate sedation is maintained throughout
- Sedation time is documented with start and end times
Do not bill this code when:
- The total sedation time is 15 minutes or less. Bill only D9239
- Deep sedation or general anesthesia time is being extended. Use D9223
- The time represents post-sedation recovery without active sedation management
- No IV sedation medications are being actively administered or titrated
Insurance and Denial Prevention
Key Payer Rules:
- D9243 must always be billed in conjunction with D9239 (the first 15 minutes)
- Some payers cap the total number of sedation increments reimbursed
- Time documentation must be precise; rounding rules vary by payer
- The same pre-authorization and medical necessity requirements as D9239 apply
Common Denials and How to Respond:
- Excessive units - Provide the sedation time log with exact start and stop times and justify the duration based on procedures performed.
- Missing D9239 - D9243 cannot be billed without D9239. Ensure D9239 is on the same claim.
- Should be D9223 - Clarify the sedation level. D9243 is for moderate sedation (patient responsive to commands); D9223 is for deep sedation/general anesthesia (patient not easily aroused).
