D03402D Cephalometric Radiographic Image - Acquisition, Measurement and Analysis
2026 Billing Guide
A cephalometric X-ray including the image capture, tracings or measurements, and written analysis for orthodontic or surgical treatment planning.
What This Code Covers
D0340 covers the full cephalometric radiograph workflow: taking the 2D cephalometric image, performing measurements or tracings, and completing a written analysis. This is primarily used in orthodontics and orthognathic (jaw) surgery planning. The ceph X-ray shows the side profile of the skull and measures relationships between teeth, jaws, and facial bones. The key difference from D0250 is that D0340 includes the analysis component, not just image capture.
Billing Guide
Bill this code when:
- A lateral cephalometric radiograph is taken and cephalometric analysis (tracings, measurements, written interpretation) is also performed
- The image and analysis are used for orthodontic diagnosis or treatment planning
- The image and analysis are used for orthognathic surgery planning
- The provider documents both the image acquisition and the analytical findings
Do not bill this code when:
- Only the cephalometric image was taken with no measurement or analysis. Use D0250 for image-only capture
- A panoramic image was taken. Use D0330
- A 3D CBCT was used for cephalometric analysis. Use the appropriate 3D imaging code
- The analysis was not documented in the patient record
Insurance and Denial Prevention
Key Payer Rules:
- Most orthodontic benefit plans cover D0340 as part of initial orthodontic records
- Some plans only cover D0340 when billed by a specialist (orthodontist or oral surgeon)
- If the orthodontic benefit has been exhausted or is not included, D0340 will likely be denied
- Medical insurance may cover D0340 when used for surgical planning of jaw deformities or sleep apnea
Common Denials and How to Respond:
- No orthodontic benefit → Verify the plan includes orthodontic coverage. If the ceph was for a medical condition, consider billing to medical insurance.
- Downgraded to D0250 → The payer may pay only for the image. Appeal with documentation showing measurement and analysis were performed.
- Provider type not eligible → Some plans restrict D0340 to specialists. Check payer credentialing requirements.
