What This Code Covers
D0250 covers a single 2D extra-oral radiographic image taken with the radiation source and detector positioned outside the patient's mouth. This includes lateral cephalometric images (without analysis), submentovertex views, posteroanterior skull films, and other 2D projections. The key distinction from D0340 is that D0250 covers image capture only, with no cephalometric measurement or analysis included.
Billing Guide
Bill this code when:
- A 2D extra-oral radiographic image is captured for diagnostic purposes
- The image is a lateral cephalometric taken for records without performing tracings or analysis
- A skull film or other 2D extra-oral projection is needed for diagnosis
- The provider captures the image but refers the analysis to another provider
Do not bill this code when:
- A cephalometric image is taken AND measurement/analysis is performed. Use D0340 instead
- A panoramic image is taken. Use D0330
- A 3D CBCT or cone beam image is taken. Use the appropriate D0360-series code
- The image is intraoral. Use D0220, D0230, or D0240
Insurance and Denial Prevention

- Most orthodontic benefit plans cover D0250 as part of initial orthodontic records
- If the patient does not have orthodontic benefits, commercial dental plans may not cover extra-oral images routinely

- Frequency exceeded → Verify the date of the last extra-oral image. If a new image is clinically needed, submit an appeal with the clinical justification.
