What This Code Covers
D3331 covers the non-surgical treatment of an obstruction within a root canal that prevents standard endodontic therapy from being completed. Common obstructions include separated (broken) endodontic instruments, calcified canals, posts, or other foreign materials lodged in the canal space. This code is billed in addition to the root canal therapy code when extra procedural time and skill are required to address the obstruction.
Billing Guide
Bill this code when:
- A separated instrument must be bypassed or removed from a root canal
- A calcified or obliterated canal requires additional effort to negotiate
- A post or other obstruction must be removed to allow retreatment
- The obstruction is addressed non-surgically (through the canal access)
Do not bill this code when:
- The canal is simply narrow but negotiable with standard technique (part of routine root canal)
- Surgical access is used to address the obstruction. Use D3410-D3427
- The obstruction is a routine part of retreatment already covered by D3346-D3348
- No actual obstruction is encountered during treatment
