D7111Extraction, Coronal Remnants - Primary Tooth
2026 Billing Guide
Removal of the remaining crown portion of a primary (baby) tooth, typically when the root has already resorbed.
What This Code Covers
D7111 covers the extraction of the remaining coronal (crown) portion of a primary (baby) tooth when the roots have significantly resorbed. This is a simpler procedure than a full erupted tooth extraction because the root structure has already dissolved as part of the natural exfoliation process. The retained crown fragment is removed to allow the permanent tooth to erupt properly or to address discomfort.
Billing Guide
Bill this code when:
- The remaining crown portion of a primary tooth is removed
- The roots have significantly resorbed (naturally or pathologically)
- The tooth fragment is retained and interfering with eruption or causing problems
- The tooth is a primary (baby) tooth, not a permanent tooth
Do not bill this code when:
- The primary tooth still has intact roots. Use D7140 for erupted tooth extraction
- The tooth is permanent. Use D7140 or D7210
- A surgical extraction is needed (tissue or bone removal required). Use D7210
- The tooth is fully erupted with intact root structure
Insurance and Denial Prevention
Key Payer Rules:
- D7111 is typically covered at a lower fee than D7140 due to the simpler procedure
- Most pediatric dental plans and Medicaid cover D7111
- Documentation of the retained fragment and resorbed roots supports the code selection
- Some payers may question why D7111 is used instead of D7140
Common Denials and How to Respond:
- Should be D7140 - Clarify that the roots had significantly resorbed and only the coronal remnant was present. Submit the radiograph showing root resorption.
- Not medically necessary - Document the clinical indication (eruption blockage, infection, discomfort).
- Wrong tooth type - Verify the tooth is primary. If permanent, use D7140 or D7210.
