What This Code Covers
D7270 covers the reimplantation and/or stabilization of a tooth that has been accidentally knocked out (avulsed) or displaced from its socket due to trauma. The tooth is reinserted into its socket and splinted (stabilized) to the adjacent teeth to allow healing and reattachment. Time is critical for reimplantation success. This code is commonly used in dental emergencies involving sports injuries, falls, or other facial trauma.
Billing Guide
Bill this code when:
- An avulsed (knocked out) tooth is reimplanted into its socket
- A displaced (luxated) tooth is repositioned and stabilized
- A splint is placed to stabilize the reimplanted or repositioned tooth
- The injury is from accidental trauma (not intentional extraction)
Do not bill this code when:
- An intentionally extracted tooth is being replanted (transplant). Use D7280
- The tooth cannot be saved and is extracted. Use D7140 or D7210
- A splint is placed for a fractured jaw without tooth reimplantation. Use D7710-D7780
- The stabilization is a periodontal splint for mobile teeth. Use D4321

Key Payer Rules:
- D7270 is typically covered as an emergency/trauma benefit
- The procedure must be related to accidental trauma (not elective)
- The splint is included in D7270 and is not billed separately
- Follow-up root canal therapy may be needed and is billed separately (D3310-D3330)

- Not an emergency - Document the traumatic event, the emergency visit, and the clinical urgency of reimplantation.
- Missing documentation - Submit detailed notes including the mechanism of injury, time from avulsion to reimplantation, and the splinting technique used.
- Splint billed separately - The splint is included in D7270. If a separate splint code was billed, withdraw it and rely on D7270 to cover both reimplantation and stabilization.
