D8040Limited Orthodontic Treatment of the Adult Dentition
2026 Billing Guide
Short-term orthodontic treatment for an adult to address a specific problem, such as pre-prosthetic alignment or minor relapse correction.
What This Code Covers
D8040 covers limited orthodontic treatment for an adult patient with permanent dentition. Treatment targets a specific orthodontic problem such as pre-prosthetic tooth movement (aligning teeth before a bridge or implant), minor relapse correction from previous orthodontic treatment, or correction of a specific tooth position. This is not comprehensive full-arch treatment.
Billing Guide
Bill this code when:
- Limited orthodontic treatment is provided to an adult patient
- Treatment addresses a defined, specific problem
- The treatment is shorter and more focused than comprehensive orthodontics
- Common indications: pre-prosthetic alignment, relapse correction, single-arch treatment
Do not bill this code when:
- Comprehensive adult orthodontic treatment is provided. Use D8090
- The patient is an adolescent. Use D8030
- The patient has mixed dentition. Use D8020
- Only a retainer is provided
Insurance and Denial Prevention
Key Payer Rules:
- Adult orthodontic coverage is less common than adolescent orthodontic coverage
- D8040 may count toward the lifetime orthodontic benefit
- Some plans exclude adult orthodontics entirely
- Pre-prosthetic orthodontics may be covered under restorative benefits in some cases
Common Denials and How to Respond:
- Adult orthodontics not covered - Many plans exclude orthodontic treatment for adults. Verify benefits before treatment begins.
- Cosmetic - Document the functional or dental health reason for treatment (tooth cannot be restored without alignment, pre-implant positioning, preventing further drift).
- Should be comprehensive - Clarify the limited scope and specific objective of treatment.
