What This Code Covers
D8220 covers fixed appliance therapy as a component of orthodontic treatment. Fixed appliances are cemented or bonded to the teeth and cannot be removed by the patient. Common examples include rapid palatal expanders (RPE), lingual holding arches, Nance appliances, trans-palatal arches, and fixed habit appliances. This code is used when the fixed appliance is the primary treatment, not part of a comprehensive braces case.
Billing Guide
Bill this code when:
- A fixed orthodontic appliance is the primary treatment
- The appliance is cemented/bonded to teeth and not removable by the patient
- Treatment objectives include expansion, space maintenance, arch development, or habit correction
- The appliance is standalone, not part of comprehensive orthodontic treatment
Do not bill this code when:
- The fixed appliance is part of comprehensive treatment (D8070-D8090) already billed
- Braces (brackets and wires) are the treatment. Use comprehensive codes D8070-D8090
- A removable appliance is used. Use D8210
- A space maintainer is placed after premature tooth loss without orthodontic treatment intent
