D8220Fixed Appliance Therapy

2026 Billing Guide

Use of a fixed (cemented) orthodontic appliance such as a space maintainer, rapid palatal expander, or lingual arch as primary treatment.

What This Code Covers

D8220 covers fixed appliance therapy as a component of orthodontic treatment. Fixed appliances are cemented or bonded to the teeth and cannot be removed by the patient. Common examples include rapid palatal expanders (RPE), lingual holding arches, Nance appliances, trans-palatal arches, and fixed habit appliances. This code is used when the fixed appliance is the primary treatment, not part of a comprehensive braces case.

Billing Guide

Bill this code when:

  • A fixed orthodontic appliance is the primary treatment
  • The appliance is cemented/bonded to teeth and not removable by the patient
  • Treatment objectives include expansion, space maintenance, arch development, or habit correction
  • The appliance is standalone, not part of comprehensive orthodontic treatment

Do not bill this code when:

  • The fixed appliance is part of comprehensive treatment (D8070-D8090) already billed
  • Braces (brackets and wires) are the treatment. Use comprehensive codes D8070-D8090
  • A removable appliance is used. Use D8210
  • A space maintainer is placed after premature tooth loss without orthodontic treatment intent

Insurance and Denial Prevention

Key Payer Rules:

  • D8220 may be covered under orthodontic or preventive benefits depending on the plan
  • Some plans cover fixed appliances (like expanders) separately from comprehensive orthodontics
  • The appliance fabrication and cementation are included in D8220
  • Pre-authorization helps clarify benefit classification

Common Denials and How to Respond:

  • Should be comprehensive treatment - Clarify that the fixed appliance is a standalone treatment addressing a specific issue, not full braces.
  • Not medically necessary - Document the crossbite, space loss, or other condition requiring the fixed appliance.
  • Lifetime benefit impact - Verify with the payer how D8220 applies to the orthodontic lifetime maximum.
