D8210Removable Appliance Therapy
2026 Billing Guide
Use of a removable orthodontic appliance (such as a retainer, expander, or space maintainer) as part of orthodontic treatment.
What This Code Covers
D8210 covers removable appliance therapy as a component of orthodontic treatment. This includes removable appliances such as retainers, removable expanders, functional appliances (Bionator, Twin Block), space maintainers, or other removable devices used to move teeth, guide jaw growth, or maintain tooth positions. This code is used when the removable appliance is the primary treatment modality, not when it is included in a comprehensive orthodontic case.
Billing Guide
Bill this code when:
- A removable orthodontic appliance is the primary treatment
- The appliance is used for active tooth movement, jaw growth modification, or space maintenance
- The treatment is distinct from comprehensive fixed appliance therapy
- Examples: removable expander, functional appliance, active retainer for tooth movement
Do not bill this code when:
- The removable appliance is part of comprehensive treatment already billed under D8070-D8090
- A fixed (cemented) appliance is used. Use D8220
- Only a retention retainer is delivered after completed treatment. Use D8680
- The appliance is a habit appliance only (thumb crib without orthodontic movement)
Insurance and Denial Prevention
Key Payer Rules:
- D8210 may be covered under orthodontic benefits or separately from comprehensive treatment
- Some plans cover removable appliance therapy even if they do not cover comprehensive orthodontics
- The appliance fabrication cost is included in D8210
- Pre-authorization helps clarify whether D8210 counts toward the orthodontic lifetime maximum
Common Denials and How to Respond:
- Not medically necessary - Document the specific problem the appliance addresses and why removable appliance therapy is the appropriate treatment.
- Should be comprehensive treatment - Clarify that the removable appliance is the sole treatment, not a component of comprehensive braces.
- Counts toward lifetime maximum - Verify with the payer how D8210 affects the orthodontic benefit and inform the patient.
