What This Code Covers
D8680 covers the orthodontic retention phase, including the removal of fixed orthodontic appliances (braces), construction and placement of retainer(s), and the initial retention period management. Retainers may be removable (Hawley, clear/Essix) or fixed (bonded lingual wire). This code marks the transition from active treatment to the retention phase that maintains the achieved tooth alignment.
Billing Guide
Bill this code when:
- Braces are removed and retainers are delivered at the completion of active treatment
- The retention phase is billed separately from the comprehensive treatment fee
- Retainer fabrication, delivery, and initial retention visits are included
- Both removable and fixed retainers are covered under this code
Do not bill this code when:
- The retention phase is included in the comprehensive treatment global fee (D8070-D8090)
- A retainer replacement is needed after the initial retention period. Use D8692 or D8693
- A removable appliance is used for active treatment (not retention). Use D8210
- Only an adjustment to an existing retainer is performed
