D8680Orthodontic Retention (Removal of Appliances, Construction and Placement of Retainer(s))

2026 Billing Guide

Removal of braces and delivery of retainers at the completion of orthodontic treatment to maintain tooth alignment.

What This Code Covers

D8680 covers the orthodontic retention phase, including the removal of fixed orthodontic appliances (braces), construction and placement of retainer(s), and the initial retention period management. Retainers may be removable (Hawley, clear/Essix) or fixed (bonded lingual wire). This code marks the transition from active treatment to the retention phase that maintains the achieved tooth alignment.

Billing Guide

Bill this code when:

  • Braces are removed and retainers are delivered at the completion of active treatment
  • The retention phase is billed separately from the comprehensive treatment fee
  • Retainer fabrication, delivery, and initial retention visits are included
  • Both removable and fixed retainers are covered under this code

Do not bill this code when:

  • The retention phase is included in the comprehensive treatment global fee (D8070-D8090)
  • A retainer replacement is needed after the initial retention period. Use D8692 or D8693
  • A removable appliance is used for active treatment (not retention). Use D8210
  • Only an adjustment to an existing retainer is performed

Insurance and Denial Prevention

Key Payer Rules:

  • In many cases, D8680 is included in the comprehensive treatment global fee (D8070-D8090) and is not billed separately
  • Some plans break orthodontic treatment into separate phases and reimburse D8680 as a distinct charge
  • Retainer replacement after the initial retention period may be a separate benefit
  • Verify with the payer how retention is handled in their fee structure

Common Denials and How to Respond:

  • Included in comprehensive fee - If the comprehensive case fee includes retention, D8680 cannot be billed separately. Verify the billing arrangement.
  • Not a covered benefit - Some plans do not cover retention separately. Check if it is bundled into the comprehensive fee.
  • Already reimbursed - If the payer has already paid the full comprehensive fee, retention is included.
