Repair of a previously placed dental sealant that has partially or completely worn off or chipped.
What This Code Covers
D1353 covers the repair of a sealant that was previously placed on a tooth. Over time, sealants can partially or completely wear away, chip, or debond. When this happens, the remaining sealant may be removed and a new sealant reapplied, or additional sealant material may be added to fill in the gaps. This code is billed per tooth.
Billing Guide
Bill this code when:
- A previously placed sealant has partially or fully lost and needs repair
- The tooth is still free of decay and only the sealant needs to be replaced
- The repair involves removing remnants of the old sealant and applying new material
- The tooth was evaluated and the sealant was found to be deficient
Do not bill this code when:
- A brand new sealant is being placed on a tooth that never had one. Use D1351
- Decay has developed under or around the failed sealant requiring a restoration
- The original sealant was placed recently and the repair is within the warranty period covered by the original code
- The payer does not recognize D1353 and requires rebilling as D1351

- Not all payers recognize D1353 as a separate code. Some require sealant repairs to be billed as D1351
- When covered, frequency limits for sealants may apply to both D1351 and D1353 combined
- Some payers have a waiting period after the original sealant before they cover a repair
