D1520Removable Unilateral Space Maintainer - Per Quadrant

2026 Billing Guide

A removable appliance that holds space in one quadrant after premature loss of baby teeth.

What This Code Covers

D1520 covers a removable space maintainer designed to hold space in one quadrant. This appliance can be taken in and out by the patient or parent. It is used when a child loses one or more baby teeth prematurely in a single quadrant and a fixed appliance is not suitable. The removable design may include an acrylic plate with wire clasps and may also incorporate a prosthetic tooth for esthetics.

Billing Guide

Bill this code when:

  • A removable space maintainer is fabricated and delivered for one quadrant
  • The child lost baby teeth prematurely in a single quadrant
  • A fixed space maintainer is not suitable (insufficient anchor teeth, patient preference)
  • The appliance is delivered and fits properly

Do not bill this code when:

  • A fixed space maintainer is placed. Use D1510
  • The appliance covers both sides. Use D1525 (maxillary) or D1526 (mandibular)
  • The appliance is orthodontic in nature and addresses alignment, not just space maintenance
  • The permanent teeth have already erupted into the space

Insurance and Denial Prevention

Key Payer Rules:

  • Coverage varies more than fixed space maintainers. Some plans prefer fixed appliances and may deny removable
  • Billed per quadrant, so if two quadrants are treated, bill D1520 twice
  • Documentation must support why a removable design was clinically indicated
  • Some plans require pre-authorization for space maintainers

Common Denials and How to Respond:

  • Fixed appliance preferred → Document why a fixed appliance was not suitable for this case.
  • Not medically necessary → Provide documentation of premature tooth loss and space loss risk.
  • Patient compliance concerns → If the payer raises compliance issues with removable appliances, note the clinical rationale for the removable design.
