D1575Distal Shoe Space Maintainer - Fixed, Unilateral
2026 Billing Guide
A space maintainer with a metal extension that slides under the gum tissue to guide an unerupted first permanent molar into position.
What This Code Covers
D1575 covers a distal shoe space maintainer, which is a specialized fixed appliance used when a primary second molar is lost before the first permanent molar has erupted. The appliance has a metal extension (shoe) that slides under the gum tissue and guides the erupting permanent molar into its correct position. This is one of the few space maintainers that is placed subgingivally (under the gum line). It requires precise placement verified by radiograph.
Billing Guide
Bill this code when:
- The primary second molar was lost or extracted before the first permanent molar erupted
- A distal shoe appliance with a subgingival extension is fabricated and placed
- Radiographic confirmation of the shoe position is obtained
- The appliance is fixed to an adjacent tooth and guides the erupting permanent molar
Do not bill this code when:
- The first permanent molar has already erupted. Use a standard band-and-loop (D1510) instead
- The appliance does not have a subgingival extension (it is a standard space maintainer)
- The appliance is removable
- The space does not require a distal shoe design
Insurance and Denial Prevention
Key Payer Rules:
- Distal shoe space maintainers are more expensive than standard space maintainers and may require pre-authorization
- The payer may request radiographic proof that the permanent molar has not yet erupted
- Coverage is typically one per site per lifetime
- Some plans may cover D1575 at the same rate as D1510
Common Denials and How to Respond:
- Permanent molar already erupted → If the payer claims the permanent molar is present, submit the pre-operative radiograph showing it was unerupted at the time of placement.
- Standard space maintainer sufficient → Explain that a distal shoe is needed because no distal abutment tooth exists for a standard band-and-loop design.
- Pre-authorization required → Submit pre-authorization with radiographs and clinical notes before placement.
