Bill this code when:

• The primary second molar was lost or extracted before the first permanent molar erupted
• A distal shoe appliance with a subgingival extension is fabricated and placed
• Radiographic confirmation of the shoe position is obtained
• The appliance is fixed to an adjacent tooth and guides the erupting permanent molar

Do not bill this code when:

• The first permanent molar has already erupted. Use a standard band-and-loop (D1510) instead
• The appliance does not have a subgingival extension (it is a standard space maintainer)
• The appliance is removable
• The space does not require a distal shoe design
