D6040Eposteal Implant

2026 Billing Guide

A dental implant that is placed on top of the jawbone beneath the gum tissue, used when the bone is too thin for conventional endosteal implants.

What This Code Covers

D6040 covers an eposteal (subperiosteal) implant, which is a custom-fabricated metal framework placed on top of the jawbone beneath the periosteum (the membrane covering the bone). Posts extend through the gum tissue to support a prosthesis. This type of implant is used in cases where the jawbone is too thin or atrophied for conventional endosteal (in-bone) implants and bone grafting is not an option. Eposteal implants are rarely placed in modern practice but remain in the CDT code set.

Billing Guide

Bill this code when:

  • An eposteal (subperiosteal) implant framework is surgically placed on the jawbone
  • The patient does not have sufficient bone height or width for endosteal implants
  • A custom metal framework is fabricated and placed under the periosteum
  • The implant is designed to support a complete or partial prosthesis

Do not bill this code when:

  • An endosteal (root-form) implant is placed into the bone. Use D6010
  • A mini implant is placed. Use D6013
  • An implant abutment is placed. Use D6056 or D6057
  • Bone grafting is performed to enable an endosteal implant. Use D7953

Insurance and Denial Prevention

Key Payer Rules:

  • D6040 is rarely billed and may not be in some payers' fee schedules
  • Many plans exclude implant procedures entirely
  • If covered, the payer may require documentation showing why conventional implants are not possible
  • Pre-authorization is strongly recommended due to the unusual nature of the procedure

Common Denials and How to Respond:

  • Not a covered benefit - Many plans exclude implants. Verify benefits before treatment.
  • Alternative treatment available - Document why bone grafting with endosteal implants or a conventional denture is not appropriate for this patient.
  • No fee schedule listing - If the payer does not have D6040 in their system, submit with detailed documentation and request manual review.
