D5140Immediate Denture - Mandibular
2026 Billing Guide
A complete lower denture fabricated before extractions and inserted immediately after teeth are removed, so the patient never goes without teeth.
What This Code Covers
D5140 covers a complete lower (mandibular) immediate denture that is fabricated before the remaining teeth are extracted and inserted on the same day as the extractions. Like the maxillary version (D5130), the denture is made in advance from pre-extraction impressions and records, and placed immediately after the lower teeth are removed. A future reline is expected as the tissues heal and change shape.
Billing Guide
Bill this code when:
- A complete mandibular denture is placed immediately following extraction of the remaining lower teeth
- The denture was fabricated in advance of the extraction appointment
- The patient's remaining lower teeth are being removed and replaced on the same day
Do not bill this code when:
- The denture is placed after healing (not immediately). Use D5120
- The denture is an upper (maxillary) immediate denture. Use D5130
- The denture is a partial, not a complete denture
- The procedure is a reline of an existing denture. Use D5750 or D5751
Insurance and Denial Prevention
Key Payer Rules:
- D5140 is covered at the same benefit level as D5120 (standard mandibular denture)
- Extraction codes are billed separately
- Frequency limits apply (typically one denture every 5-10 years)
- The reline after healing is a separate covered procedure
Common Denials and How to Respond:
- Frequency limit - Verify the plan's replacement frequency and when the last denture was placed.
- Should be D5120 - Clarify the denture was placed immediately on the same day as extractions, not after a healing period.
- Missing extraction documentation - Submit the extraction codes and radiographs on the same claim.
