What This Code Covers

D5411 covers the adjustment of a complete mandibular (lower) denture to improve its fit, comfort, or function. Lower dentures are particularly prone to sore spots and instability due to less surface area and tongue movement. Adjustments include relieving pressure points, adjusting the occlusion, and modifying flanges for comfort and better retention.

Billing Guide

Bill this code when:

• A complete lower denture requires chairside adjustment
• Sore spots, pressure points, or occlusal discrepancies are corrected
• Border or flange adjustments are made for comfort or retention
• The adjustment is beyond the post-delivery inclusion period

Do not bill this code when:

• A partial denture is adjusted. Use D5422 for mandibular partial adjustment
• A reline is performed. Use D5750 or D5751
• The adjustment is part of the initial delivery and follow-up period
• The denture needs replacement rather than adjustment

Insurance and Denial Prevention

Key Payer Rules:

• Same rules as D5410 apply for mandibular adjustments
• Post-delivery adjustments within 30-90 days are typically included in the denture fee
• After the inclusion period, D5411 is separately billable
• Some plans limit the number of adjustments per year

Common Denials and How to Respond:

• Within post-delivery period - Verify the plan's included follow-up period. Bill after that period ends.
• Frequency limit - Document the specific clinical need for each adjustment visit.
• Wrong code - D5411 is for a complete mandibular denture. If the patient has a partial denture, use D5422.
