What This Code Covers
D5421 covers the chairside adjustment of a removable maxillary (upper) partial denture. Adjustments may include relieving sore spots on the tissue-bearing surfaces, adjusting clasps for better retention or comfort, modifying the occlusion (bite), or smoothing rough areas. This is a maintenance procedure performed after the initial delivery and follow-up period.
Billing Guide
Bill this code when:
- An existing upper partial denture requires chairside adjustment
- Clasp adjustment, sore spot relief, or occlusal corrections are performed
- The adjustment is beyond the initial post-delivery follow-up period
- The denture is a removable partial (not a complete denture)
Do not bill this code when:
- A complete upper denture is adjusted. Use D5410
- A lower partial denture is adjusted. Use D5422
- A reline is performed. Use D5750 or D5751
- The adjustment is within the post-delivery inclusion period
Insurance and Denial Prevention
Key Payer Rules:
- D5421 is a low-cost covered benefit on most plans
- Post-delivery adjustments (30-90 days) are typically included in the partial denture fee
