Each carrier, and often different plan types within the same carrier, may route paper claims to different processing centers based on the plan's geographic region, employer group, or product line. The correct claims address is typically found on the back of the patient's insurance card, in the carrier's provider manual, or through the carrier's provider portal. Using an incorrect or outdated address can result in claims being lost, returned, or significantly delayed in processing.

While electronic claim submission has become the industry standard, paper claims remain necessary in certain situations. Some smaller carriers or government programs may not accept electronic submissions for all claim types. Claims requiring physical attachments such as original radiographs, photographs, or specialty referral documents may also need to be mailed in paper form. Additionally, claim corrections or appeals that require original signatures or supporting documentation may need to be submitted via mail.
