A plaster or stone model of a patient's teeth created from dental impressions, used for diagnosis, treatment planning, and documentation of oral conditions.

Diagnostic casts, also referred to as study models, are three-dimensional reproductions of a patient's dental arches made by pouring dental stone or plaster into impression molds. These models allow clinicians to examine tooth alignment, occlusion, arch form, and spatial relationships outside the patient's mouth. They are commonly used in orthodontics, prosthodontics, and complex restorative treatment planning where a thorough analysis of the dentition is required before initiating care.

In the context of dental billing, diagnostic casts have dedicated CDT procedure codes and are billable when they serve a distinct diagnostic or treatment planning purpose. The cost of materials, lab time, and clinical time to take the impression should be captured through proper coding. Many insurance carriers require diagnostic casts as part of the pre-authorization package for orthodontic treatment, full-mouth rehabilitation, or removable prosthodontics. Without these models, the supporting documentation may be deemed incomplete, leading to delays or denials of the treatment authorization.

The shift toward digital impressions and 3D-printed models has changed the workflow but not the billing principles. Digital diagnostic models created from intraoral scans are billed under the same coding framework as traditional poured casts. Practices should ensure their fee schedules reflect the cost of either workflow and that the clinical record documents the purpose of the diagnostic cast. Retaining casts or their digital equivalents is also important for medicolegal documentation and for demonstrating treatment progress over time. Offices that routinely fail to bill for diagnostic casts leave revenue on the table, particularly in orthodontic and prosthodontic specialties where these models are standard components of the diagnostic workup.
