A full-mouth X-ray, commonly abbreviated as FMX, is a complete set of intraoral radiographs that captures every tooth, root, and the surrounding bone structures. A typical FMX consists of 18 to 20 individual images, including periapical views (showing individual teeth from crown to root tip) and bitewing views (showing the crowns of upper and lower teeth together to detect interproximal cavities).

The FMX is considered the gold standard for complete radiographic evaluation. It provides detailed information that a panoramic X-ray alone cannot capture, including early interproximal cavities, periapical infections, bone levels around each tooth, root morphology, and the condition of existing restorations. It is typically recommended for new patients, patients who have not had X-rays in several years, and patients with extensive dental needs.

For practices, proper protocols around FMX are important for both clinical and administrative reasons. The ADA recommends radiographic intervals based on individual patient risk, not on a fixed schedule. Prescribing X-rays based on clinical need rather than routine ensures compliance with guidelines and protects the practice. From a billing standpoint, the FMX is coded under CDT code D0210 and is generally covered by insurance once every three to five years, depending on the plan. Having a clear policy for when an FMX is indicated versus when a panoramic film or updated bitewings will suffice helps standardize care and avoid unnecessary radiation exposure.
