D0251 covers an extra-oral posterior dental radiographic image. This is similar to a traditional bitewing X-ray but captured with the sensor or film positioned outside the patient's mouth. This technique is used when the patient cannot tolerate intraoral film placement due to a strong gag reflex, limited mouth opening, or other physical limitations. The image captures the posterior teeth for caries detection and bone level assessment.
Billing Guide
Bill this code when:
- A posterior dental image is captured using extra-oral technique because the patient cannot tolerate intraoral film
- The patient has a strong gag reflex, trismus, or physical limitation preventing intraoral bitewing placement
- The extra-oral posterior image is used for caries detection or periodontal bone level assessment
- The image is taken per exposure, similar to intraoral bitewing billing
Do not bill this code when:
- Standard intraoral bitewings (D0272, D0274) can be successfully taken
- A panoramic image (D0330) is taken instead, which also shows posterior teeth
- The image is an intraoral periapical or bitewing. Use the appropriate D0220-D0274 codes
- A CBCT is used for posterior evaluation. Use D0360-series codes
