D0251Extra-oral Posterior Dental Radiographic Image

2026 Billing Guide

A posterior bitewing-type image captured using an extra-oral sensor or film positioned outside the mouth.

What This Code Covers

D0251 covers an extra-oral posterior dental radiographic image. This is similar to a traditional bitewing X-ray but captured with the sensor or film positioned outside the patient's mouth. This technique is used when the patient cannot tolerate intraoral film placement due to a strong gag reflex, limited mouth opening, or other physical limitations. The image captures the posterior teeth for caries detection and bone level assessment.

Billing Guide

Bill this code when:

  • A posterior dental image is captured using extra-oral technique because the patient cannot tolerate intraoral film
  • The patient has a strong gag reflex, trismus, or physical limitation preventing intraoral bitewing placement
  • The extra-oral posterior image is used for caries detection or periodontal bone level assessment
  • The image is taken per exposure, similar to intraoral bitewing billing

Do not bill this code when:

  • Standard intraoral bitewings (D0272, D0274) can be successfully taken
  • A panoramic image (D0330) is taken instead, which also shows posterior teeth
  • The image is an intraoral periapical or bitewing. Use the appropriate D0220-D0274 codes
  • A CBCT is used for posterior evaluation. Use D0360-series codes

Insurance and Denial Prevention

Key Payer Rules:

  • D0251 is a relatively newer code and some payers may not yet have it mapped in their fee schedules
  • Reimbursement is typically comparable to intraoral bitewing codes (D0272/D0274)
  • Documentation of why extra-oral technique was required is essential for claim approval
  • Some payers may request a narrative explaining the patient's inability to tolerate intraoral films

Common Denials and How to Respond:

  • Code not recognized → Some older systems may not recognize D0251. Contact the payer to confirm code mapping. If not recognized, discuss alternative billing with the payer.
  • Not medically necessary → Submit documentation explaining the clinical reason for extra-oral technique (gag reflex, trismus, physical limitation).
  • Downgraded to intraoral bitewing → If the payer pays at a lower rate, accept or appeal with documentation showing extra-oral technique was the only viable option.
