What This Code Covers
D0470 covers diagnostic casts (study models) made from impressions of the patient's upper and lower arches. The impressions are poured in stone to create physical models that the dentist uses for treatment planning, orthodontic analysis, occlusal evaluation, and fabrication planning for prosthetics or appliances. Digital models created from intraoral scans may also qualify under this code when used for diagnostic purposes.
Billing Guide
Bill this code when:
- Impressions or digital scans are taken specifically for diagnostic study models
- The models are used for orthodontic case analysis, occlusal assessment, or treatment planning
- The casts are needed to plan prosthetic work, implant cases, or complex restorative treatment
- The models are a separate diagnostic service, not part of fabricating an appliance or prosthesis
Do not bill this code when:
- The impressions are taken as part of fabricating a crown, denture, or other prosthesis (those impressions are included in the prosthetic code)
- The models are for lab use only and do not serve a diagnostic purpose
- The models are duplicates of previously made study casts with no new clinical need
- Digital scans are taken solely for same-day crown milling (CAD/CAM), which is part of the restorative procedure
Insurance and Denial Prevention
Key Payer Rules:
- Most orthodontic plans cover D0470 as part of initial orthodontic records
- Commercial dental plans may not cover D0470 for general diagnostic purposes
- Some payers consider diagnostic casts bundled into the comprehensive evaluation or the prosthetic procedure
- Frequency limits are generally one set per treatment case
