Bill this code when:
- Gingivectomy or gingivoplasty is performed on four or more contiguous teeth in one quadrant

- The indication is periodontal, not cosmetic crown lengthening
Do not bill this code when:
- Fewer than four contiguous teeth are treated. Use D4211 for one to three teeth
