What This Code Covers
D4265 covers the application of biologic materials (such as enamel matrix derivatives, platelet-rich fibrin, growth factors, or other biologics) to aid in soft tissue and osseous (bone) tissue regeneration during periodontal surgery. These materials enhance the body's natural healing response and promote regeneration of periodontal tissues including bone, cementum, and periodontal ligament. This code is billed in addition to the surgical code.
Billing Guide
Bill this code when:
- Biologic regenerative materials are applied during periodontal surgery
- Materials such as enamel matrix derivative (Emdogain), PRP/PRF, or recombinant growth factors are used
- The material is applied to promote tissue regeneration, not just to fill a defect
- The procedure is performed as an adjunct to osseous surgery or flap procedures
Do not bill this code when:
- Bone graft material is placed without biologic agents. Use D4263 for bone grafts
- A barrier membrane is placed. Use D4266 for guided tissue regeneration
