What This Code Covers
D4270 covers a pedicle soft tissue graft procedure where gum tissue adjacent to a recession site is surgically repositioned (rotated or laterally moved) to cover an exposed root surface. Unlike free gingival grafts or connective tissue grafts that harvest tissue from a distant donor site (palate), a pedicle graft uses tissue that remains attached to its blood supply at one end, swinging it over to cover the neighboring area of recession. This code is billed per site.
Billing Guide
Bill this code when:
- A pedicle graft (laterally positioned flap, coronally advanced flap with pedicle) is performed
- Adjacent tissue is repositioned while maintaining its blood supply attachment
- The goal is to cover an exposed root surface or augment gum tissue
- The tissue is from the same surgical area, not a remote donor site
Do not bill this code when:
- A free soft tissue graft is harvested from the palate or other remote site. Use D4273 or D4274
- A connective tissue graft from a remote donor site is used. Use D4273
- An allograft (donor tissue from tissue bank) is used. Use D4275
- The tissue repositioning is part of a flap procedure for pocket reduction (included in D4240/D4260)
