D1110 covers a routine dental cleaning (prophylaxis) for adult patients, typically age 14 and older. The hygienist removes plaque, calculus, and stains from all tooth surfaces using hand instruments, ultrasonic scalers, or both, followed by polishing. This is a preventive service for patients with a healthy periodontium or gingivitis only, not for patients with active periodontal disease or a history of periodontal treatment. D1110 is almost always billed alongside D0120 and may include fluoride application (D1206).

Billing Guide

Bill this code when:

  • Adult patient (14+) presents for a routine dental cleaning
  • Patient does not have active periodontal disease or a history of periodontal treatment
  • Probing depths are generally within normal limits (1-3mm)
  • Cleaning is preventive in nature, not therapeutic

Do not bill this code when:

  • Patient has a history of periodontal treatment (scaling and root planing, surgery). Use D4910 (periodontal maintenance)
  • Patient has clinical signs of periodontitis (4mm+ pockets, bone loss). Use D4341 or D4342 (scaling and root planing)
  • Patient is under the payer's age threshold for adult prophy. Use D1120 (child prophylaxis)
  • Frequency limit exceeded without medical necessity documentation. Most plans cover two per year
