Billing Guide
Bill this code when:
- An all-ceramic crown is placed directly on the implant body (implant-supported)
- No separate abutment is used between the implant and crown
- The crown is typically screw-retained to the implant
- The restoration is a single-tooth implant crown
Do not bill this code when:
- A separate abutment supports the crown. Use D6058 for all-ceramic on abutment
- The crown is porcelain fused to metal. Use D6066
- The crown is on a natural tooth. Use D2740
- An abutment code is also being billed for the same implant (abutment-supported is D6058)
