D6065 covers an all-porcelain or all-ceramic crown that is supported directly by a dental implant body, without a separate abutment component. This is typically a one-piece screw-retained restoration where the crown is fabricated with an integrated implant connection and screwed directly into the implant. This approach eliminates the separate abutment and its associated cost, and is commonly used for single-tooth implant restorations.

Billing Guide

Bill this code when:

• An all-ceramic crown is placed directly on the implant body (implant-supported)
• No separate abutment is used between the implant and crown
• The crown is typically screw-retained to the implant
• The restoration is a single-tooth implant crown

Do not bill this code when:

• A separate abutment supports the crown. Use D6058 for all-ceramic on abutment
• The crown is porcelain fused to metal. Use D6066
• The crown is on a natural tooth. Use D2740
• An abutment code is also being billed for the same implant (abutment-supported is D6058)

Insurance and Denial Prevention

Key Payer Rules:

• D6065 is a single code covering the implant-supported crown without a separate abutment
• Some payers may not distinguish between D6065 and D6058 in their fee schedules
• Since no abutment is billed, the total cost may be lower than the abutment + crown combination
• Pre-authorization is recommended for implant restorations
