D6058 covers an all-porcelain or all-ceramic crown that is supported by an implant abutment. This is a two-piece implant restoration where the crown sits on top of a separate abutment (D6050, D6056, or D6057) that is attached to the implant body. All-ceramic crowns offer the best esthetics and are commonly used in the visible front teeth area. The crown is cemented or screw-retained onto the abutment.

Billing Guide

Bill this code when:

  • An all-porcelain or all-ceramic crown is placed on an implant abutment
  • The restoration is abutment-supported (two-piece: abutment plus crown)
  • The crown material is entirely ceramic with no metal substructure
  • The abutment is already in place or being placed at the same visit

Do not bill this code when:

  • The crown is porcelain fused to metal (PFM). Use D6059
  • The crown is placed directly on the implant body without a separate abutment. Use D6065 or D6066
  • The crown is on a natural tooth, not an implant. Use D2740
  • The abutment is being billed. Bill the abutment code separately (D6050, D6056)
