This is a two-piece restoration where the crown sits on a separate abutment attached to the implant.
Billing Guide
Bill this code when:
- A PFM crown with high noble metal substructure is placed on an implant abutment
- The restoration is abutment-supported (two-piece)
- The metal substructure contains over 60% noble metals (at least 40% gold)
- The abutment is already in place or being placed at the same visit
Do not bill this code when:
- The crown is all-ceramic. Use D6058
- The metal is predominantly base metal. Different code applies
- The crown sits directly on the implant body without a separate abutment. Use D6065 or D6066
- The crown is on a natural tooth. Use D2750
