D6066 covers a porcelain-fused-to-metal (PFM) crown with a high noble metal substructure that connects directly to the implant body without a separate abutment. Like D6065, this is an implant-supported (not abutment-supported) restoration, but with a PFM material instead of all-ceramic. The high noble metal substructure (over 60% noble metals, at least 40% gold) provides excellent strength and biocompatibility.
Billing Guide
Bill this code when:
- A PFM crown with high noble metal is placed directly on the implant body
- No separate abutment is used
- The crown is implant-supported (typically screw-retained)
- The metal substructure is high noble (over 60% noble metals)
Do not bill this code when:
- The crown is all-ceramic on the implant body. Use D6065
- A separate abutment supports the crown. Use D6059 for abutment-supported PFM
- The crown is on a natural tooth. Use D2750
- The metal is base metal or noble (not high noble)
