Bill this code when:
- Decay or damage involves exactly two surfaces of the same tooth and amalgam is placed
- The two surfaces are contiguous and restored in a single procedure
- The tooth being restored is either a primary or permanent tooth
- Amalgam (silver) is the material selected by the provider
Do not bill this code when:
- Only one surface is involved. Use D2140 for a single-surface amalgam
- Three or more surfaces are restored. Use D2160 for three surfaces or D2161 for four or more
- Composite resin is used instead of amalgam. Use D2392 for a two-surface posterior composite
- Two separate single-surface fillings are placed on the same tooth on different surfaces that are not connected. Bill two units of D2140 only if the restorations are truly independent
