D2335Resin-Based Composite - Four or More Surfaces or Involving Incisal Angle, Anterior
2026 Billing Guide
A large tooth-colored composite filling on a front tooth involving four or more surfaces or the biting edge.
What This Code Covers
D2335 covers a large resin-based composite filling on an anterior tooth that involves four or more surfaces or includes the incisal angle (the corner of the biting edge). This is one of the most common codes for extensive front-tooth repairs from trauma, decay, or replacing old restorations. The incisal angle involvement is the key distinguishing factor from D2332.
Billing Guide
Bill this code when:
- A composite filling on an anterior tooth involves four or more surfaces
- The restoration involves the incisal angle (corner edge of the tooth)
- The tooth is an incisor or canine (teeth 6-11, 22-27)
- The filling rebuilds a significant portion of the visible tooth structure
Do not bill this code when:
- Three or fewer surfaces without incisal angle involvement. Use D2330, D2331, or D2332
- The tooth needs a full composite crown. Use D2390
- A lab-made crown or veneer is more appropriate. Use D2740 or the veneer code
- The tooth is posterior
Insurance and Denial Prevention
Key Payer Rules:
- Most plans cover D2335 at the anterior composite benefit level
- Some payers may question whether a crown would be more appropriate for such extensive restoration
- Frequency limits apply per tooth and surface combination
- Pre-existing condition clauses may apply if the damage occurred before coverage began
Common Denials and How to Respond:
- Crown recommended → Document why a composite restoration is appropriate (sufficient remaining structure, patient preference, age of patient).
- Incisal angle not documented → Clearly note in the chart and on the claim that the incisal angle was involved.
- Pre-existing condition → If the damage occurred after coverage began, provide documentation of the date of injury or onset of decay.
