D2799Provisional Crown - Further Treatment or Completion of Diagnosis Necessary Prior to Final Impression
2026 Billing Guide
A temporary crown placed while waiting for the final crown or while completing additional diagnostic work.
What This Code Covers
D2799 covers a provisional (temporary) crown placed when additional treatment or diagnostic evaluation is needed before the final crown can be fabricated. This is not the standard temporary crown placed between the prep and seat appointments (which is included in the final crown code). D2799 is for situations where the temporary crown needs to stay in place for an extended period while other treatment is completed, such as endodontic therapy, periodontal treatment, or occlusal stabilization.
Billing Guide
Bill this code when:
- A provisional crown is needed for an extended period before the final crown can be placed
- Additional treatment (root canal, perio surgery, implant healing) must be completed first
- The provisional crown is a separate procedure from the eventual final crown
- The clinical situation requires diagnostic evaluation before committing to a final restoration
Do not bill this code when:
- The temporary crown is the standard interim restoration between crown prep and seat (included in the final crown code)
- A sedative/temporary filling is placed. Use D2940
- The final crown is being placed at this visit
- No clinical reason exists for delaying the final crown
Insurance and Denial Prevention
Key Payer Rules:
- Many payers do not cover D2799 separately, considering it part of the crown process
- When covered, reimbursement is typically modest compared to a final crown
- The payer may require documentation that the delay is clinically necessary
- D2799 should not be billed if the final crown is being billed at the same time
Common Denials and How to Respond:
- Included in final crown → Document that the provisional is a separate service due to the need for additional treatment before the final crown.
- Not medically necessary → Provide the clinical reason for the extended provisional period (endo treatment, perio healing, diagnostic evaluation).
- Timing issue → If the payer questions the gap between provisional and final, explain the treatment sequence.
