D2950Core Buildup, Including Any Pins When Required

2026 Billing Guide

Building up the core structure of a tooth to provide adequate support for a crown, one of the most commonly billed crown-related codes.

What This Code Covers

D2950 covers building up the core of a tooth using composite, amalgam, or other restorative material to provide adequate structure for a crown. This is necessary when too much natural tooth structure has been lost to decay, fracture, or previous restorations. The buildup may include retention pins drilled into dentin for additional support. D2950 is one of the most commonly billed codes alongside crown procedures.

Billing Guide

Bill this code when:

  • A core buildup is necessary to support a crown
  • More than 50% of the coronal tooth structure is missing or compromised
  • Retention pins are placed as part of the buildup (included in the code)
  • The buildup is performed on the same date as or prior to the crown preparation

Do not bill this code when:

  • The tooth has adequate structure to support a crown without a buildup
  • The core buildup is a minor amount of material used to fill an undercut during crown prep (this is considered part of the crown procedure)
  • A post is placed. Use D2954 for a prefabricated post and core
  • The buildup is on a tooth that is not receiving a crown

Insurance and Denial Prevention

Key Payer Rules:

  • D2950 is typically covered when billed with a crown code, but some plans bundle it
  • Payers may deny D2950 if the clinical notes do not document sufficient tooth structure loss
  • Some plans require that more than 50% of the coronal structure be missing to justify a buildup
  • D2950 and D2954 should not be billed on the same tooth on the same date

Common Denials and How to Respond:

  • Bundled with crown → Some payers include the buildup in the crown fee. Appeal with documentation showing the extensive tooth structure loss that required a separate buildup procedure.
  • Not medically necessary → Submit radiographs and clinical notes showing significant loss of tooth structure.
  • Missing documentation → Document the percentage of missing tooth structure and the clinical need for the buildup.
