Bill this code when:

- The restoration is a direct composite placed chairside, not a lab-fabricated piece
- Both surfaces are restored in the same visit as one continuous restoration
Do not bill this code when:
- Only one surface is involved. Use D2330 for a single-surface anterior composite
- Three or more surfaces are restored. Use D2332 for three surfaces or D2335 for four or more surfaces on an anterior tooth
- The tooth is a premolar or molar. Use D2392 for a two-surface posterior composite
- An inlay or onlay is placed instead of a direct filling. Use the appropriate indirect restoration code (D2651 or similar)
