D3110Pulp Cap - Direct (Excluding Final Restoration)
2026 Billing Guide
Placement of a protective material directly on an exposed pulp to preserve tooth vitality, billed separately from the final restoration.
What This Code Covers
D3110 covers the placement of a biocompatible material (such as calcium hydroxide or MTA) directly on exposed pulp tissue to encourage healing and preserve tooth vitality. This is performed when the pulp is accidentally or minimally exposed during caries removal or trauma. The final restoration is billed separately.
Billing Guide
Bill this code when:
- The pulp is visibly exposed during caries excavation or trauma
- A direct pulp capping material is placed on the exposure site
- The goal is to maintain pulp vitality and avoid root canal therapy
- The final restoration (filling or crown) is billed as a separate code
Do not bill this code when:
- No pulp exposure occurred. Use D3120 for indirect pulp cap
- A pulpotomy is performed instead. Use D3220 (primary) or D3221 (permanent)
- The tooth requires root canal therapy due to irreversible pulpitis
- The pulp cap material is placed as part of a sedative filling. Use D2940
Insurance and Denial Prevention
Key Payer Rules:
- D3110 is typically covered as a basic or endodontic benefit
- The final restoration must be billed separately (not included in D3110)
- Some payers require documentation of visible pulp exposure
- Coverage may be limited to one direct pulp cap per tooth
Common Denials and How to Respond:
- No documentation of pulp exposure - Submit clinical notes describing the exposure and the material placed directly on the pulp tissue.
- Bundled with restoration - D3110 is a separate endodontic procedure. Appeal with documentation showing it is distinct from the restoration.
- Should be D3120 - D3120 is for indirect pulp cap where no exposure occurred. Clarify that the pulp was visibly exposed.
