D3120Pulp Cap - Indirect (Excluding Final Restoration)
2026 Billing Guide
Placement of a protective liner over nearly exposed pulp to protect it and preserve tooth vitality, billed separately from the final restoration.
What This Code Covers
D3120 covers the placement of a protective liner or base material over a thin layer of remaining dentin near the pulp, without actual pulp exposure. This is used during deep caries removal when the dentist intentionally leaves a small amount of affected dentin to avoid exposing the pulp, then places a biocompatible material to encourage reparative dentin formation. The final restoration is billed separately.
Billing Guide
Bill this code when:
- Deep caries is removed and the pulp is nearly exposed but not visibly exposed
- A protective liner (calcium hydroxide, glass ionomer, MTA) is placed over the thin dentin floor
- The procedure is performed as a deliberate vital pulp therapy strategy
- The final restoration is billed as a separate code
Do not bill this code when:
- The pulp is visibly exposed. Use D3110 for direct pulp cap
- A routine cavity liner is placed as part of a standard restoration (included in the restoration code)
- A sedative or temporary filling is placed. Use D2940
- The tooth requires pulpotomy or root canal therapy
Insurance and Denial Prevention
Key Payer Rules:
- D3120 is typically covered as a basic or endodontic benefit
- Some payers consider a routine liner to be included in the restoration and will deny D3120 unless deep caries near the pulp is clearly documented
- Radiographic evidence of deep caries is often required for approval
- The final restoration must be billed separately
Common Denials and How to Respond:
- Bundled with restoration - A routine liner is part of the restoration, but D3120 is a separate vital pulp therapy procedure. Appeal with radiographs showing deep caries near the pulp and clinical notes describing the deliberate pulp protection step.
- Not medically necessary - Submit the pre-operative radiograph showing caries extending close to the pulp chamber.
- Should be D3110 - Clarify that the pulp was not exposed. D3110 requires visible pulp exposure; D3120 is for near-exposure situations.
